Privacy Policy
Supporting You With Transparency
1. Introduction
Alliance Residences (“Alliance Residences,” “we,” “us,” or “our”) operates assisted living communities, including Rosewood Haven in Champlin, Minnesota and Tulip Terrace in Maple Grove, Minnesota, and the website allianceresidences.com (the “Site”). We respect your privacy and are committed to protecting the personal information you share with us.
This Privacy Policy explains what information we collect, how we use and share it, and the choices you have. It applies to information collected through our Site, our online forms, our social media advertising (including Facebook and Instagram lead forms), phone and email inquiries, and in-person interactions at our communities.
Please read this Policy carefully. By using our Site or submitting information to us, you agree to the practices described here. If you do not agree, please do not use the Site or submit your information.
2. Information We Collect
We collect information in the following ways:
Information you provide directly. When you request a tour, submit a referral, contact us, or respond to one of our advertisements, we may collect your name, email address, phone number, mailing address, your relationship to a prospective resident, the prospective resident’s name and general information, your preferred contact method and timing, and any other details you choose to share in a message or form.
Referral and health-related information. If you are a caseworker, discharge planner, family member, or other party submitting a referral through our referral form, you may provide health-related and sensitive information about a prospective resident, which can include date of birth, insurance or payer details, diagnosis, care needs, and similar information. We treat this information with heightened care as described in Section 8 (Health Information and HIPAA).
Information collected automatically. When you visit our Site, we and our service providers may automatically collect technical information such as your IP address, browser type, device type, pages viewed, links clicked, referring website, and the dates and times of your visits. We collect this information using cookies and similar technologies (see Section 6).
Information from advertising platforms. When you interact with our ads on platforms such as Facebook or Instagram — including submitting a “lead form” directly within those platforms — we receive the information you provide in that form (such as your name, email, and phone number) and limited information about your interaction with the ad. This information is provided to us by the platform in accordance with the platform’s terms and its own privacy policy.
3. How We Use Your Information
We use the information we collect to:
- Respond to your inquiries, schedule tours, and follow up with you about our communities;
- Process and follow up on referrals and evaluate prospective residents’ care needs;
- Provide, operate, personalize, and improve our Site and services;
- Communicate with you about our communities, availability, events, and related updates you have requested;
- Measure and improve the performance of our advertising and marketing;
- Maintain the safety and security of our Site, and detect and prevent fraud or misuse;
- Comply with applicable laws, regulations, and legal processes, and enforce our agreements.
We will only contact you for marketing purposes where permitted by law, and you may opt out at any time (see Section 7).
4. How We Share Your Information
We do not sell your personal information. We may share your information in the following limited circumstances:
Service providers. We share information with third parties that perform services on our behalf, such as website hosting, form processing, customer relationship management, email delivery, and advertising and analytics. These providers are permitted to use your information only to perform services for us.
Within our organization. We may share your information among Alliance Residences communities and affiliated entities in order to respond to your inquiry and coordinate care and admissions.
Legal and safety. We may disclose information when we believe it is reasonably necessary to comply with a law, regulation, subpoena, or legal process; to protect the safety, rights, or property of any person; or in connection with a merger, acquisition, financing, or sale of assets.
With your direction. We may share your information with third parties when you ask us to, or when you provide it for a purpose that necessarily involves such sharing (for example, when a referral source submits information on behalf of a prospective resident).
5. Advertising and Social Media Platforms
We advertise our communities on third-party platforms, including Meta (Facebook and Instagram). When you engage with our ads or submit a lead form on those platforms, the platform collects your information under its own privacy policy and provides certain information to us so we can follow up with you.
We may also use tools such as the Meta Pixel and similar technologies to understand how our ads perform and to reach people who may be interested in our communities. These platforms may combine the information they collect about you across their services in accordance with their own policies. To understand and control how these platforms use your information, please review their privacy settings and policies, including Meta’s Privacy Policy and ad preferences.
6. Cookies and Tracking Technologies
Our Site uses cookies and similar technologies to operate the Site, remember your preferences, analyze traffic, and measure and improve our advertising. Cookies are small files stored on your device.
You can control cookies through your browser settings, including deleting existing cookies and declining new ones. Some features of the Site may not function properly if you disable cookies. Where required by law, we will request your consent before placing non-essential cookies.
We do not knowingly respond to “Do Not Track” browser signals at this time. If that changes, we will update this Policy.
7. Your Choices
Marketing communications. You may opt out of marketing emails by using the unsubscribe link in any such email, or by contacting us using the details in Section 12. You may ask us to stop calling or texting you at any time. Even if you opt out of marketing messages, we may still contact you about an inquiry or referral you submitted.
Access, correction, and deletion. Subject to applicable law, you may request access to, correction of, or deletion of the personal information we hold about you. To make a request, contact us using the details in Section 12. We may need to verify your identity before acting on your request.
Your state privacy rights. Depending on where you live, you may have additional rights under state privacy laws regarding your personal information. We will honor applicable rights and will not discriminate against you for exercising them.
8. Health Information and HIPAA
Some information submitted to us — particularly through our referral process — may include health-related information about a prospective resident. To the extent Alliance Residences maintains “protected health information” as a HIPAA-covered entity or business associate, that information is handled in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and our separate Notice of Privacy Practices, which governs that information and controls over this Policy where they conflict.
If you are submitting information about another person, you represent that you are authorized to provide that information for the purpose of a referral or inquiry. Please do not submit another person’s sensitive health information unless you have the authority to do so.
9. Data Security
We maintain reasonable administrative, technical, and physical safeguards designed to protect your information against unauthorized access, use, or disclosure. However, no method of transmission over the internet or method of electronic storage is completely secure, and we cannot guarantee absolute security.
10. Data Retention
We retain your information for as long as necessary to fulfill the purposes described in this Policy, to respond to your inquiries, to comply with our legal obligations, to resolve disputes, and to enforce our agreements. When information is no longer needed, we take reasonable steps to delete or de-identify it.
11. Children’s Privacy
Our Site and services are intended for adults. We do not knowingly collect personal information from children under 13. If you believe a child has provided us with personal information, please contact us and we will take appropriate steps to delete it.
