Position Summary
The Registered Nurse (RN) / Clinical Supervisor is the clinical leader of our assisted living community. The RN conducts resident assessments, develops person-centered care plans, delegates and supervises nursing tasks to unlicensed personnel, and partners with the leadership team to ensure safe, high-quality, regulation-compliant care. This role combines hands-on clinical work with team leadership and is ideal for an RN who enjoys mentoring staff and shaping the standard of care.
Essential Duties and Responsibilities
Resident Assessment and Care Planning
- Complete initial, 14-day, 90-day, annual, and change-of-condition nursing assessments per MDH 144G requirements.
- Develop and update individualized, person-centered care plans aligned with physician orders, resident goals, and family input.
- Coordinate care with providers, hospice, home health, hospitals, and community partners.
- Lead care conferences with residents, families, and the interdisciplinary team.
Delegation and Supervision
- Formally delegate nursing tasks to unlicensed personnel with documented training and competency evaluations.
- Conduct supervisory visits and ongoing competency reviews of delegated tasks.
- Mentor, coach, and develop the LPN and DSP team, including the Lead DSPs.
- Provide clinical guidance and on-call support to floor staff.
Medication and Treatment Oversight
- Oversee medication management practices, including pharmacy audits and controlled substance reconciliation.
- Review physician orders for accuracy and follow-through.
- Monitor outcomes of treatments, interventions, and medication changes.
Compliance, Quality, and Documentation
- Maintain compliance with Minnesota Department of Health (MDH) 144G regulations, Vulnerable Adults Act, and all applicable state and federal laws.
- Lead survey readiness, quality assurance, and performance improvement initiatives.
- Support implementation, training, and troubleshooting of the EHR and clinical software platforms.
- Participate in policy review, infection control oversight, and incident investigations.
Required Qualifications
- Current, unencumbered Registered Nurse (RN) license in the State of Minnesota.
- Current Basic Life Support (BLS) / CPR certification.
- Working knowledge of Minnesota assisted living statutes (MDH 144G) and the Vulnerable Adults Act.
- Strong assessment, clinical judgment, and care planning skills.
- Ability to pass a Minnesota DHS background study.
Preferred Qualifications
- 2+ years of RN experience, with experience in assisted living, memory care, long-term care, home health, or geriatrics strongly preferred.
- Prior supervisory, charge nurse, or clinical leadership experience.
- BSN preferred.
- Experience with EHR implementation, training, or super-user roles.
Required Training
- MDH 144G orientation prior to providing direct resident care.
- 8 hours of dementia care training within the first 160 working hours, and at least 2 hours annually thereafter.
- Mandated reporter training under the Minnesota Vulnerable Adults Act.
- Ongoing continuing education to maintain RN licensure.
Physical Requirements
- Ability to assess residents, perform clinical interventions, and assist with transfers as needed.
- Ability to lift up to 50 lbs (approximately 22 kg) and assist with two-person transfers when required.
- Frequent standing, walking, and bending; extended periods of computer work for documentation.
- Ability to respond on-site or by phone during designated on-call periods.

Schedule
Alliance Residences offers full-time, part-time, and PRN opportunities across day, evening, and overnight (NOC) shifts, including weekends and holidays. Specific shift availability and scheduling expectations will be reviewed during the interview process.
