Licensed Practical Nurse (LPN) / Floor Charge Nurse

Position Summary

The Licensed Practical Nurse (LPN) / Floor Charge Nurse delivers skilled nursing care within the Minnesota LPN scope of practice and serves as the clinical lead on the floor during the shift. The LPN performs treatments, oversees medication administration, supervises Direct Support Professionals, and works closely with the RN Clinical Supervisor to keep residents safe, comfortable, and well cared for.

Essential Duties and Responsibilities

Clinical Care

  • Perform nursing treatments within the Minnesota LPN scope of practice, including wound care, catheter care, ostomy care, blood glucose monitoring, and insulin administration.
  • Administer medications and treatments accurately per physician orders and facility policy.
  • Conduct routine clinical observations and report acute changes in condition to the RN Clinical Supervisor promptly.
  • Provide compassionate, person-centered care that respects resident dignity and preferences.

Medication and Order Management

  • Audit medication administration records (MARs) for accuracy and follow-through.
  • Receive and process pharmacy deliveries, including narcotic counts and storage per policy.
  • Transcribe physician orders and route them for RN review and approval.
  • Communicate with pharmacy, providers, and families regarding medication and treatment updates.

Shift Leadership and Supervision

  • Serve as the on-shift charge nurse, coordinating clinical care delivery across the team.
  • Provide direction and oversight to Direct Support Professionals, including reinforcing safety protocols and delegated task practices.
  • Lead emergency response on the floor, partnering with the RN on call as needed.

Compliance and Infection Control

  • Enforce facility infection-control policies and standard precautions.
  • Maintain accurate, timely documentation in the EHR consistent with MDH 144G requirements.
  • Support survey readiness and quality assurance initiatives.

Required Qualifications

  • Current, unencumbered Licensed Practical Nurse (LPN) license in the State of Minnesota.
  • Current Basic Life Support (BLS) / CPR certification.
  • Ability to pass a Minnesota DHS background study.
  • Strong clinical judgment, time management, and communication skills.

Preferred Qualifications

  • 1+ years of LPN experience in assisted living, memory care, long-term care, or skilled nursing.
  • Experience supervising or mentoring unlicensed personnel.
  • Wound care, IV therapy, or dementia care experience.
  • Familiarity with Minnesota 144G assisted living regulations.

Required Training (Provided or Verified by Alliance Residences)

  • MDH 144G orientation, including the Assisted Living Bill of Rights, vulnerable adult reporting, and person-centered planning, prior to providing direct resident care.
  • 8 hours of dementia care training within the first 160 working hours, and at least 2 hours annually thereafter.
  • Maltreatment reporting and mandated reporter training.
  • Annual competency and infection control updates.

Physical Requirements

  • Ability to safely perform clinical interventions, transfers, and repositioning, including use of mechanical lifts.
  • Ability to lift up to 50 lbs (approximately 22 kg) and assist with two-person transfers as needed.
  • Frequent standing, walking, bending, and reaching throughout the shift.
  • Manual dexterity for medication administration, wound care, and documentation.

Schedule

Alliance Residences offers full-time, part-time, and PRN opportunities across day, evening, and overnight (NOC) shifts, including weekends and holidays. Specific shift availability and scheduling expectations will be reviewed during the interview process.
